The Diary CarePro is a mobile care management platform and workflow tool, designed for care coordinators to promote patient participation in their care plans and automate care coordination.

CarePro enables users to create care plan tasks for themselves, their colleagues, and patients, and prioritize these tasks using the built-in task sharing system and calendar. Reminders are automatically sent to patients for tasks including attending appointments, taking medications, collecting prescriptions, and recording vital signs. CarePro interfaces with The Diary app for patients, which enables patients to record their vitals, and receive notifications and reminders through their smartphone. Assessments can be sent to patients, including onboarding surveys, depression screening (PHQ-9) assessments, and alcohol use disorders identification test (AUDIT). Two-way video conferencing and secure messaging can also be used to communicate with patients in real-time.

CarePro allows users to set up custom thresholds for patient measurements, which notify users via SMS, email, or web notification when data entered violates any of the thresholds. Data recorded by patients is visible to users in real-time, enabling users to remotely monitor care plan adherence, minimize risks, and modify care plans if necessary. Patient and population metrics can be analyzed and reports generated, including productivity indicators, outcomes-to-cost comparisons, services completed per patient population, and referral, utilization, and billing reports.
