Health Cloud by Salesforce is a cloud-based healthcare CRM and patient management solution which enables collaboration between healthcare providers, payers, and patients. The software is built on Salesforce’s CRM platform and specifically designed to meet the needs of the healthcare industry.

Health Cloud Empower enables care providers, including care coordinators, hospice care, home nurses, and patients’ family members, to engage with care teams using any internet-enabled device. Care Team Lists allow caregivers to view and communicate with care teams, the Care Plan Selector enables the assignment of care plans to patients by caregivers, and Care Plan Tasks let care providers assign tasks to patients on-the-go. Permissions can be managed to control access to patient data, ensuring personal medical information is kept secure.

Concurrent Care Plans allow multiple teams with disparate expertise and workflows to collaborate and build relationships across care teams. Care providers can assign multiple care plans for comorbid patients to different care teams to provide more personalized care journeys. Health Cloud also automatically calculates patient risk scores based on the Center for Medicaid Services Hierarchical Condition Category (CMS-HCC) model, allowing providers to view risk across patient populations. The customizable risk dashboards give providers insight into the populations they serve, and enable the provision of targeted preventative care to the patients most in need.

    Meets the needs of multiple departments across provider organizations. Highly customizable and robust data capabilities.

    Cons

    Integration out of the box is somewhat limited but anything can be built. Storage costs & licensing.

    One of the best health service cloud. Beautiful SaaS.

    Pros

    1. Ease of use and Design. We don't have to train people to use Health cloud.
    2. Very Scalable and many sales force features can be embedded.

    Cons

    I felt a price is bit high as compared to SaaS products. More functionality like auto scrapping would be really useful anyday.

    I feel comfortable recommending this product to any office. I think they would be able to use the templates and customization to suit the offices' needs.

    Pros

    The format s easier to read, less eye strain. The colors are easy to distinguish from the font color. I spend a lot of the time in front of the computer.

    Cons

    almost too cute icons, Some of the transitions have these little cartoons. Maybe this is targeting the younger workforce.
