Lumeon is a cloud-based patient engagement platform that helps healthcare organizations manage appointments, treatment plans, and conversations across the entire patient journey. Features include decision support, virtual check-in, referrals, auto-updates, resource allocation, and milestone tracking.

The application enables clinicians to send automated reminders to patients via a preferred communication channel such as email, voice and text messages, conduct video conferences, and test different message types to predict the possibility of no-shows. Lumeon lets team members automate care operations through risk-based task distribution, auto-escalation, health tracking, and assessment capabilities. Health centers can also outreach patients through HIPAA-compliant campaigns, gain insights on patients' health by collecting data through eForms, and follow-up on appointments.

Lumeon allows consultants to track progress in real-time through the dashboard and check patients' discharge capability using questionnaires, checklists, or protocols. Patients can self-schedule appointments based on available slots and receive notifications for being on waitlists. It also offers integration with various third-party electronic health records (EHR) applications, including Epic and Cerner.
