1) Only after hitting submit does the page give the option to upload docs/pics. 
2) After hitting submit the JOB COMPLETION button shows up and ...yes, I have hit that button by mistake - operator error :-(.  Once I caught my mistake and the other time I did not, and the request went into the completion column. 
There was a \#3, but I forgot what it was - in between being interrupted while trying to complete this.
